Malaria: fever after a risk zone means test today.
Malaria symptoms can start weeks after the bite. The single rule that saves lives: fever within months of visiting a malaria zone means same-day testing, not waiting.
PLATE 201
- Tuck under the mattress: no gaps at the edge
- Dusk ritual: Anopheles bite evenings and nights
The clock from bite to test
The bite
Anopheles bite at night. If you slept without a net in a malaria zone, the clock starts here.
Silent incubation
The parasite multiplies in the liver. You feel nothing. Some species stay silent for months.
Fever with chills
Cyclical fever, shaking chills, sweating. This is the test trigger, not a wait signal.
Blood test
A thick smear or rapid test answers in under an hour. Treatment depends on species and region.
Red flags: any one means same-day care
Especially dangerous: children under five, pregnancy, and travelers returning from high-transmission areas.
Fever after a risk zone: three questions
Work through them in order; any answer pointing to malaria means a test today.
Malaria maps change by district and season. Check the destination, not the country name.
Within months still counts. Some species incubate quietly for a year.
That combination is the test trigger. Same-day blood test, tell the clinician the itinerary.
THE QUESTION PEOPLE ACTUALLY TYPE
How long after the bite do malaria symptoms start? Usually 7 to 30 days, but some species stay silent for months. Any fever after visiting a malaria zone means test the same day, even long after returning.

Where the 2023 burden actually sat
Malaria is not evenly spread. Five countries carried more than half of estimated cases. The map that matters is district-level; this table is the first cut.
| COUNTRY / REGION | SHARE OF 2023 ESTIMATES |
|---|---|
| WHO African Region | 94% of cases, 95% of deaths |
| Nigeria | 26% of global cases |
| Democratic Republic of the Congo | 13% of global cases |
| Uganda, Ethiopia, Mozambique | about 5%, 4% and 4% |
| Children under five | about 76% of malaria deaths |
